Required Medical Forms for Participation

Freshman and transfer student-athletes must have a physical exam within six months of the first day of the athletic season they are participating in.

Physical exams are valid for 365 days from the date of the exam.  All returning student-athletes must have a valid physical on file in order to participate in Athletics at Mount Ida College.
